Diving is an exciting and fun activity and, when performed correctly, applying correct techniques, it is relatively safe. However, diving can also be physically strenuous under certain conditions. The linked Medical Statements allow you to determine whether you should consult your physician prior to engaging in dive activities.

A positive response to the questions on the applicable form does not necessarily disqualify you from diving. Rather, a positive response means that there is a preexisting condition that may affect your safety while diving and you must seek the advice of your physician prior to engaging in dive activities.

Read the applicable Statement carefully prior to signing. If you are a minor, you must have this Statement signed by a parent or guardian. Statements should be downloaded, printed, filled out, and signed prior to arriving for your SCUBA experience. If necessary, a physician certification should also be obtained prior to arriving for your SCUBA experience.
